Sun Damage: An Overview & How We Treat It with Topical Bio-Actives

In my clinic, I treat a lot of clients with sun damage — and my go-to bio-actives are always the same powerful combination: retinol, antioxidants, peptides, humectants, and emollients. The skin responds beautifully to this protocol, and the results speak for themselves.

After more than 33 years of hands-on clinical experience, I want to be honest with you: we cannot completely reverse the effects of sun damage. But we can meaningfully improve the health and appearance of your skin. I tell my clients this is a journey — and results absolutely happen when you're consistent with your at-home skincare protocol.

That said, I cannot emphasize this enough: sun protection is non-negotiable. Skin improvement will not occur if sun safety isn't part of your daily routine. I'm not being harsh — I'm being honest. 80–90% of photodamage comes from UV radiation. Minimize your exposure, and you will begin to see real improvement in your skin's texture, tone, and resilience.

What Is Photoaging?

Sun damage or photoaging is the premature aging of your skin caused by ongoing exposure to ultraviolet radiation from the sun, tanning beds, or even fluorescent lighting. This is distinctly different from chronological aging, which is driven by time and genetics.

UV radiation doesn't just accelerate aging through oxidative stress at the cellular level — it also alters the DNA of skin cells, causing mutations that can increase your risk of developing skin cancer. While DNA damage at the cellular level cannot be reversed, the visible effects of UV-induced skin damage can be significantly improved with the right topical bio-actives: retinol, peptides, antioxidants, emollients, and humectants.


Risk Factors for Photoaging

Biological & Genetic Factors

  • Fitzpatrick Skin Types I & II — fair skin, light hair, light eyes — burn easily or always burn. With less melanin for natural UV defense, these skin types are most vulnerable to photoaging.
  • Fitzpatrick Skin Types IV–VI — darker skin tones that rarely to never burn — carry more melanin, which offers greater natural UV protection. However, photodamage, burns, and skin cancer can still occur. Photodamage in deeper skin tones often presents as stubborn, uneven hyperpigmentation that can be difficult to treat.

Unprotected UVR Exposure & Where You Live

  • Constant UV exposure: It's not just one bad sunburn — it's a lifetime of UV exposure that quietly degrades collagen and elastin within the skin's extracellular matrix.
  • Geography: If you live at a higher altitude or closer to the equator, you're receiving a higher dose of UV radiation every single day — whether you feel it or not.
  • Recreational habits: Sporadic, intense sun exposure — a weekend at the beach, an afternoon on the golf course — is actually more damaging than consistent, low-level exposure. Your skin doesn't get a chance to adapt.

Environmental & Lifestyle Triggers

  • Smoking: The toxins in cigarette smoke trigger oxidative stress and actively destroy the collagen network that keeps skin firm and lifted — accelerating sagging and premature aging. 
  • Air pollution: Microscopic particulate matter amplifies oxidative damage and speeds up wrinkle formation.
  • Nutrition: What you eat shows up on your skin. A diet high in refined sugars and ultra-processed foods breaks down collagen from the inside out. A diet rich in antioxidants — colorful fruits, leafy greens, healthy fats, and lean proteins — actively works to protect and repair it.

Signs of Photoaging

Signs can begin as early as your teens and twenties:

  • Broken capillaries (nose, cheeks, neck, chest)
  • Redness and blotchiness
  • Fine lines and wrinkles
  • Rough, uneven, or leathery skin texture
  • Actinic keratosis (red, rough, scaly spots)
  • Pigment changes and hyperpigmentation
  • Thinning skin and loss of elasticity (actinic elastosis)
  • Lip lesions (actinic cheilitis)
  • Skin cancers

What Causes Photoaging?

UV radiation alters the DNA of skin cells, accelerating premature aging. There are two types of UV radiation that harm the skin:

UVA rays are longer wavelengths that penetrate through the epidermis and deep into the dermis — our true skin, composed of collagen, elastin, fibroblasts, blood vessels, and lymphatic vessels. UVA damage degrades collagen and elastin, resulting in wrinkles, lines, and loss of firmness.

UVB rays are shorter wavelengths that penetrate the epidermis, causing sunburn, redness, dehydration, and pigment changes. UVB rays are responsible for the majority of skin cancers, including basal cell carcinoma, squamous cell carcinoma, and melanoma.


How We Treat Photoaging Topically

  • Retinol — Stimulates collagen production in the dermis and accelerates new skin cell turnover, promoting firmer, thicker skin.
  • Vitamin C & Antioxidants — Defend against reactive oxygen species and oxidative stress, repair cellular damage, and reduce inflammation.
  • Peptides — Promote collagen synthesis, repair the extracellular matrix, shield against UV radiation, and inhibit melanogenesis.
  • Alpha Hydroxy Acids (AHAs) — Exfoliate to promote cellular turnover and collagen production; also provide humectant and antioxidant benefits.
  • Humectants & Emollients — Support and repair the skin barrier, reduce inflammation, and deliver deep hydration and moisture.

Prevention: UV Safety Rules for You & Your Family

  • Apply a physical sunblock containing titanium dioxide or zinc oxide to all exposed areas every 3–4 hours
  • Use 1 tsp (5 ml) for face, neck, and chest or 30 ml for the entire body
  • Wear a wide-brimmed hat, sunglasses, and protective clothing
  • Seek shade whenever possible
  • Reapply after swimming or sweating — sunblocks are not waterproof
  • Remember: UV radiation penetrates glass and clouds
  • Avoid tanning beds
  • Avoid peak UV hours: 10:00 AM – 4:00 PM
